Clinical Observation of Three Dimension Conformal Radiotherapy with Concurrent Chemoradiotherapy for Middle and Advanced Cervical Cancer

Abstract
ObjectiveTo evaluate the effect and toxic reaction of three dimensional conformable radiotherapy (3D-CRT)after whole pelvic radiotherapy with concurrent chemoradiotherapy on middle and advanced cervical cancer. MethodsSixty-two patients with middle or advanced cervical cancer were divided randomly into 3D-CRT after whole pelvic radiotherapy with concurrent chemoradiotherapy(A group)and afterloading irradiation after whole pelvic radiotherapy with concurrent chemoradiotherapy(B group). After receiving 36~40Gy /20 fractions of external radiation to the whole pelvic, patients in the group A were irradiated by 3-DCRT with the total dose of 66~75Gy. Patients in the group B received 45~50Gy external radiation to the parametrrium, and point A received 30Gy by after loading irradiation (4~7Gy per-fraction,1~2 fractions per week). All patients were given concurrent chemoradiotherapy in the firstweek and the fourth week. ResultsEffective rate was 96.88% in group A and 76.67% in group B(P<0.05).The rates of bone marrow inhibition and long-term complication for the rectum and bladder were significantly lower in group A than in group B(P<0.05). Conclusion The 3D-CRT after whole pelvic radio-therapy with concurrent chemoradiotherapy for middle and advanced cervical cancer could improve local control rate.
