顺铂的时辰化疗

摘要: 目的 观察按时辰给药能否减轻顺铂（cisplatin,PDD）的毒副反应。方法 自2004年4月～2006年3月以时辰给药方式治疗29例肿瘤患者（A组）,化疗方案主要有NVB/PDD;PDD/CF/5-Fu;Taxotere/PDD。药物输注采用法国Aguettant公司生产的“Melodie”4通道程控输液泵,以正弦曲线方式给药。PDD20mg/（m^2.d）,自10∶00～22∶00,峰值在16∶00,d1～4;同期30例含PDD方案化疗的患者为对照（B组）,对照组PDD25mg/（m2.d）,自09∶00～10∶00,d1～3,常规输注。两组病例均同样采用5-HT3受体拮抗剂＋地塞米松止吐,同样水化、利尿处理。结果 A组共完成72个周期的化疗,平均每个病例完成2.48个周期。两组（A∶B）毒副反应的发生率分别为：急性呕吐13.79%∶53.33%,P〈0.001;延迟呕吐3.45%∶43.33%,P〈0.001;恶心31.03%∶63.33%,P〈0.05。血液学毒性和肝肾毒性两组无差异。结论 时辰给药在控制PDD的胃肠道反应方面具有显著性优势,从而在化疗期间带来更好的生活质量,值得临床进一步推广使用。Abstract: Objective To investigate whether chronotherapy can reduce the toxicities caused by cisplatin. Methods From April 2004 to March 2006,29 cases received cisplatin-based regimens by multichannel, programmable pumps as group A. Cisplatin was given in 20 mg/(m2·d), from 10∶00 to 22∶00,peak at 16∶00,d1~4; The other 30 cases received same regimens as group B, Cisplatin was given in 25 mg/(m2·d), from 09∶00 to 10∶00, d1～3.
