不同药物雾化吸入预防食管癌术后肺部并发症的疗效观察

Curative Effect of Different Medicine Aerosol Inhalation on Preventing Pulmonary Complication after Esophagectomy

  • 摘要: 目的 观察食管癌患者术后应用不同药物雾化吸入预防肺部并发症的效果。方法 将120 例全麻下食管癌术后患者随机分为两组:地塞米松、庆大霉素和糜蛋白酶联合雾化吸入治疗组(A 组),氨溴索和特布他林联合雾化吸入治疗组(B组)。监测患者术后(2、24、48及72 h)动脉血气指标(SaO2、PaO2和PaCO2)、气道分泌物清除效果及临床疗效,随访患者转出ICU后胸管拔除时间和术后肺部并发症(PPCs)的发生情况。结果 患者动脉血气指标、气道分泌物清除效果、呼吸系统症状改善效果、胸管拔除时间及PPCs的发生率,B组均优于A组,差异均有统计学意义(P<0.05)。结论 食管癌全麻术后患者,使用氨溴索和特布他林联合雾化吸入有助于气道分泌物清除和呼吸系统症状改善,并能缩短胸管拔除时间,减少肺部并发症发生。

     

    Abstract: Objective To compare the curative effect of different medicine aerosol inhalation on preventing pulmonary complication after esophagectomy. Methods Selected 120 patients after esophagectomy were randomly divided into two groups, Group A treated by dexamethasone, gentamicin and α-chymotrypsin combined with aerosol inhalation, Group B treated by ambroxol and terbutaline combined with aerosol inhalation. Indexes of arterial blood gases, such as SaO2, PaO2 and PaCO2, at 2, 24, 48 and 72 h after extubation, effect of airway secretions clearance and improvement of respiratory symptoms were evaluated. Extubation time after leaving ICU and occurrence of postoperative pulmonary complications (PPCs) were followed up. Results Indexes of arterial blood gases, effect of airway secretions clearance, improvement of respiratory symptoms, extubation time and incidence of PPCs in Group B were improved more significantly than those in Group A (P<0.05). Conclusion The treatment of ambroxol and terbutaline combined aerosol inhalation could improve airway secretions clearance and respiratory symptoms, shorten extubation time, and reduce postoperative pulmonary complications on esophagus cancer patients after esophagectomy.
