甲状腺癌NIS和TSHR表达的矛盾性及非相关性

Contradiction and Non-correlation between NIS and TSHR Expression in Human Thyroid Cancer

  • 摘要: 目的研究钠/碘转运蛋白(sodium/iodide symporter, NIS)和促甲状腺激素受体(thyroid stimulating hormone receptor, TSHR)在甲状腺癌中的表达和相关性。方法收集正常甲状腺组织18例、甲状腺癌23例,通过荧光定量分析和免疫组织化学SP法对NIS和TSHR的表达进行研究。结果甲状腺癌NIS 和TSHR mRNA的表达显著低于正常甲状腺组织(P<0.005),正常甲状腺组织NIS 和TSHR的表达呈显著正相关(r=0.667, P=0.000),而在甲状腺癌中无相关性(r=0.222,P=0.376)。免疫组织化学结果表明甲状腺癌NIS阳性产物主要定位于细胞质,TSHR阳性产物定位于细胞质和细胞膜,甲状腺癌NIS及TSHR蛋白阳性表达率显著高于正常组(P<0.05);甲状腺癌NIS mRNA表达下降同时伴有细胞质NIS蛋白表达升高的有17例(74%)。结论甲状腺癌NIS和TSHR表达具有非相关性以及在转录和翻译水平上具有矛盾性,NIS蛋白非功能定位于细胞质,因而不能发挥正常的运输及聚碘功能,我们推测这可能是造成甲状腺癌患者放射性碘治疗失败的重要原因。

     

    Abstract: ObjectiveTo investigate the correlation between sodium/iodide symporter and thyroid stimulating hormone receptor expression in human thyroid cancer . Methods mRNA levels, quantified by real-time PCR, and protein evaluated by immunohistochemistry, were examined in surgical specimens of 18 normal thyroid tissues and 23 thyroid carcinoma tissues. ResultsThe NIS and TSHR mRNA levels of thyroid carcinoma were significantly lower than those of normal thyroid tissue and nodular goiter(P<0.005). In addition, there was a significant positive correlation between NIS and TSHR in normal thyroid samples (r=0.667, P=0.000), but no such correlation in thyroid carcinomas(r=0.222,P=0.376). The immunohistochemistry results showed that the NIS protein mainly located in cytoplasm, and TSHR protein mainly located in cytoplasm and cytomembrane .In thyroid carcinomas, the positive rates of NIS and TSHR protein were markedly higher than those in normal thyroid tissues(P<0.05). Through combined analysis of NIS mRNA concentrations and immunostaining scores, in the thyroid carcinomas, reduction of NIS mRNA expression with increment of intracellular protein was found in 74% (17/23) of the samples compared with normal tissues. ConclusionIn thyroid carcinomas, there were expressed contradiction and non-correlation between NIS and TSHR expression. NIS was not able to play the role of protein transporting and absorbing iodine as mainly located in cytoplasm, which was regarded as an important reason of radioiodine therapy failure for thyroid carcinoma.
