肾错构瘤实时超声造影表现及定量分析

Characteristics and Quantitative Analysis of Renal Hamartoma with Real-time Contrast-enhanced Ultrasonography

  • 摘要: 目的 探讨超声造影对肾错构瘤的诊断价值。方法 对31例肾错构瘤行SonoVue实时超声造影检查,观察病灶内灌注过程及回声变化,并运用时间-强度曲线分析软件对储存的动态图像进行定量分析。结果 与周边正常肾皮质对比,肾错构瘤在始增时间、强化程度及强化持续时间上均表现出一定的特点;在时间-强度曲线上两者在灌注起始端均较圆滑,曲线较低平,相对来说,肾错构瘤弧度较低,正常肾皮质弧度较高。两者在峰值强度、达峰时间及曲线下面积上差异有统计学意义(P<0.05),曲线尖度两者比较差异无统计学意义(P>0.05)。结论 运用超声造影及时间-强度曲线定量分析技术,能更好观察并定量分析肾错构瘤的血流灌注特点,具有重要诊断价值。

     

    Abstract: Abstract:Objective To explore the value of contrast-enhanced ultrasonography(CEUS) in the diagnosis of renal hamartoma. Methods Thirty-one cases of renal hamartoma were examined with real-time CEUS SonoVue and CnTI techniques, and the perfusion process and the variation of agent was observed.The whole dynamic perfusion process on CEUS was recorded, the time-intensity curve was quantitatively analysed using software QontraXt. Results Compared with the cortex of the kidney, renal hamartoma showed special features in three aspects: the beginning time, degree and persistence time of enhancement. The time-intensity curves were slick,low and flat, but in general speaking, the curve radian of renal hamartoma was lower while normal renal cortex was higher. From the view of ultrasonographic quantified parameters, there were significant differences in the peak of the signal intensity,the time-to-peak and the areas under the curve(P<0.05), and no difference in the sharpness of the curve between renal hamartoma and normal renal cortex. Conclusion Using the CEUS and the quantitative analysis technique of time-intensity curve, we could precisely quantitatively analyse the dynamic perfusion of renal hamartoma, which is very helpful for the diagnosis and differential diagnosis of renal hamartoma.
