肺癌患者化疗致肌肉关节疼痛特点(附93例观察)

Characteristics of Pain of Muscle and Joint in Patients with Advanced Lung Cancer During Chemotherapy (93 Cases Report)

  • 摘要: 目的:观察晚期肺癌患者化疗期间肌肉、关节疼痛的特点,探讨其诱发因素、发病机制及防治方法。方法:观察93例经病理学或细胞学确诊的肺癌患者,71例应用双药含铂方案化疗、22例应用非含铂方案化疗,按性别、年龄、病理类型、化疗方案分组进行分析。对出现肌肉、关节疼痛的病例给予葡萄糖酸钙静脉输注、并观察其疗效。结果:应用双药含铂方案化疗的71例患者中29例出现肌肉、关节疼痛症状,42例未出现;22例非含铂方案化疗者均未出现。疼痛发生率与性别、年龄、病理类型均无关系,与何种化疗药物与铂类联合亦无关,但含铂方案与非含铂方案比较差别有统计学意义。29例出现肌肉、关节疼痛的病例给予葡萄糖酸钙静脉输注,6天后均取得缓解。结论:肌肉、关节疼痛与应用铂类药物有关。钙剂缓解肌肉、关节疼痛的机制仍不十分明确,有待进一步研究;但静脉补充葡萄糖酸钙治疗化疗引起的肌肉、关节疼痛有显著疗效,可以提高患者的生活质量。

     

    Abstract: Objective:To observe the characteristics of the pain of muscle and joint in patients with lung cancer during chemotherapy, define its potential inducers, explore its mechanism, and probe into the prevention and therapeutic ways. Methods:Ninety-three patients with lung cancer diagnosed by pathology or cytology were enrolled into study. Seventy-one cases were treated with double-drug chemotherapy regimen based on platinum,other 22 cases were treated with chemotherapy regimen without platinum. Stratification analysis was performed based on sex, age, type of pathology and chemotherapy regimen. Patients with muscle and joint pain were given calcium gluconate infusion. Results:Muscle and joint pain was found in 29 cases out of 71 cases treated with double-drug chemotherapy regimen including platinum agent while 22 cases treated with non-platinum regimen have no the similar symptoms. The incidence of muscle and joint pain was not correlated with gender, age and type of pathology. For chemotherapy regimen,the incidence was irrelative with which drugs platinum combined with, but there was significant difference between regimen including platinum agent and other regimens. The pain of muscle and joint was relieved in 29 cases after 6 days of intravenous infusion of calcium gluconate. Conclusion:Muscle and joint pain may be correlated with platinum agents. The mechanism of the relief of muscle and joint pain by calcium still keeps unclear. However, intravenous infusion of calcium gluconate has convincing effect on descending muscle and joint pain. This method can improve the life quality of patients.
