99Tcm-HL91 SPECT乏氧显像在非小细胞肺癌放疗中的疗效评价

Clinical Application of 99Tcm-HL91 Hypoxia Imaging in Radiotherapy of Patients with Non-small Cell Lung Cancer

  • 摘要: 目的 探讨99Tcm-4,9-二氮-3,3,10,10-四甲基十二烷-2,11-二酮肟(HL91)乏氧显像对非小细胞肺癌放疗近期疗效的评价。 方法 31例拟接受三维适形放射治疗的非小细胞肺癌患者,在放疗前2天、后2天分别静脉注射99Tcm-HL91 740-925MBq后进行4h及24h SPECT断层显像,根据显像进行目测和半定量分析(T/N值,靶与非靶比值),根据放疗前24h显像T/N值将患者分为低摄取组(T/N<2.76,n=6)及高摄取组(T/N≥2.76,n=25),对两组放疗近期疗效进行比较。 结果 低摄取组放疗近期有效率为83.3%(1/6)高于高摄取组为44.0%(11/25),两组比较差异有统计学意义(P<0.05)。两组24h断层显像T/N比值下降率分别为30.3%和13.3%,两组比较差异有统计学意义(P<0.05)。结论 99Tcm-HL91非小细胞肺癌乏氧显像在预测放疗近期效果方面具有一定的临床价值。

     

    Abstract: Objective To detect hypoxia status of non-small cell lung cancer with serial 99Tcm -4,9-diaza-3,3,10,10-tetramethyldodecan-2,11-dione dioxime (HL91) SPECT images and predict the short-time curative effect about radiotherapy in lung cancer. Methods Thirty-one patients with non-small cell lung cancer underwent SPECT imaging at 4h and 24h after injecting 99Tcm-HL91,respectively.Regions of interest(R0Is)were drawn in the tumor(T)and contralateral position(N)and the radioactivity ratios of tumor to normal(T/N)were calculated semiquantitatively.Then patients treated by three-dimensional conformal radiotherapy(3D-CRT)underwent 99Tcm-HL91 hypoxia imaging two days before and after radiotherapy,respectively.Patients were divided into low intake group and high intake group according to the T/N value( 24h) before radiotherapy,the changes of T/N value after radiotherapy was analyzed on the transaxial view. Results The short-time effective power of radiotherapy patients in low intake group were higher than that in high intake group(P<0.05).There was a significantly statistical difference between these groups(P<0.05). Conclusion Technetium-99m-labeled HL91 hypoxia imaging of lung cancer can be used to predict its short-time curative effect after radiotherapy.
