单肺通气方式对食管癌患者围手术期炎性细胞因子和氧自由基的影响

Effect of Different Modes of One Lung Ventilation on Inflammatory Factors and Oxygen Free Radicals in Patients Undergoing Esophagectomy

  • 摘要: 目的: 探讨单肺通气(OLV)方式对食管癌患者围手术期炎性细胞因子和氧自由基的影响。方法:拟行食管癌根治术患者40例,随机分为长时间组(Ⅰ组)和间断双肺通气组(Ⅱ组),每组20例。两组患者分别在麻醉诱导后(T1)、OLV 45min(T2)、90min(T3)及术后2 h(T4) 采取外周静脉血,分别测定血清肿瘤坏死因子-α(TNF-α)、白细胞介素(IL)-6、IL-8、丙二醛(MDA)浓度及超氧化物歧化酶(SOD)活性。结果:两组患者的TNF-α、IL-6和IL-8 于T2时均明显上升(P<0.05),Ⅱ组T3、T4时TNF-α、IL-6和IL-8均明显低于Ⅰ组(P<0.05 )。两组患者的MDA于T3、T4时显著升高(P<0.05),且Ⅱ组在T3、T4时显著低于Ⅰ组(P<0.05);两组患者的SOD于T3、T4时显著降低(P<0.05),且Ⅱ组在T3、T4时显著高于Ⅰ组(P<0.05)。结论:单肺通气期间间断双肺通气可减轻对肺组织的损伤。

     

    Abstract: Objective:To investigate the effect of the different modes of one lung ventilation (OLV) on the changes of inflammatory factors and oxygen free radicals in patients undergoing esophagectomy. Methods:40 patients with esophageal cancer undergoing esophagectomy were randomly divided into two groups: long time of OLV group (groupⅠ) and intermittent two lung ventilation (group Ⅱ). Blood samples were collected after induction of anesthesia(T1), at OLV 45 minutes(T2), OLV 90 minutes(T3) and 2 hours after operation (T4) for detecting TNF-α, IL-6, IL-8, MDA and SOD concentrations, respectively. Results:Serum TNF-α, IL-6 and IL-8 concentrations were all significantly increased at T2 (P<0.05). Serum TNF-α, IL-6 and IL-8 concentrations were lower at T3-4 in group Ⅱ than those in groupⅠ (P<0.05).Meanwhile, serum MDA concentration was obviously higher in T3-4 (P<0.05 )than that in T1 of both groups, and lower at T3-4 in group Ⅱ than that in groupⅠ(P<0.05).Serum SOD concentration was dramatically lower in T3-4 (P<0.05)than that in T1 of both groups, and higher at T3-4 in group Ⅱ than that in group Ⅰ(P<0.05). Conclusion :Intermittent two lung ventilation could alleviate the injury of lung during OLV through inhibiting the generation of oxygen free radicals and the release of inflammatory factors.
