癌性厌食的病理机制与诊疗进展

Pathogenic Mechanisms and Advances in Diagnosis and Treatment of Cancer-Related Anorexia

  • 摘要: 癌性厌食(CRA)是恶性肿瘤常见且严重的并发症,以持续性食欲减退、早饱及摄食减少为核心特征,可直接导致患者营养不良、治疗耐受性降低、生活质量下降及预后恶化。其发生机制复杂,涉及下丘脑摄食中枢调控失衡、外周食欲激素失调、全身炎症介质及肿瘤衍生因子作用等多层面网络化交互,并显著受到化疗、放疗及手术等抗肿瘤治疗本身的影响。当前临床管理以孕激素类药物、糖皮质激素、促胃肠动力药为基础,联合营养支持、康复锻炼、心理干预及中医药治疗的多模式综合治疗为主导,但仍缺乏可根本逆转病程的高效手段。本文系统综述CRA的病理生理机制、临床评估工具及综合治疗策略,并结合最新研究进展探讨其未来研究方向,以期为临床实践优化与新型干预策略开发提供理论依据和参考。

     

    Abstract: Cancer-related anorexia (CRA) is a common and severe complication of malignant tumors. It is characterized by persistent appetite loss, early satiety, and reduced food intake. It directly leads to malnutrition, decreased treatment tolerance, reduced quality of life, and worsened prognosis. Its pathogenesis is complex, involving a multilayered network of interactions, including the dysregulation of the hypothalamic feeding center, imbalance of peripheral appetite hormones and systemic inflammatory mediators, and actions of tumor-derived factors. It is also highly influenced by anticancer treatments, such as chemotherapy, radiotherapy, and surgery. Current clinical management is dominated by multimodal comprehensive therapy based on progestogens, glucocorticoids and prokinetic agents, combined with nutritional support, rehabilitation exercise, psychological intervention and traditional Chinese medicine. However, highly effective approaches capable of fundamentally reversing the disease course of CRA are still lacking. This article provides a systematic review of the pathophysiological mechanisms, clinical assessment tools, and comprehensive treatment strategies for CRA. It also presents a discussion on future research directions based on the latest advances to provide a theoretical basis and reference for optimizing clinical practice and developing novel intervention strategies.
