ATP Bioluminescence Technical Guides Intra-peritoneal Chemotherapy in Patients with Colorectal Cancer

  • 摘要: 目的观察ATP生物荧光技术指导大肠癌患者腹腔化疗的疗效及安全性。方法选择32例腹水癌细胞阳性的晚期大肠癌患者,ATP-TCA法检测腹水癌细胞对5-氟尿嘧啶、顺铂、羟基喜树碱、丝裂霉素、卡铂、多西他赛和吡柔比星的敏感度,选择敏感药物对患者进行腹腔内化疗,观察治疗后腹水缓解率、生活质量评分及化疗不良反应等。结果大肠癌腹水标本的药物敏感度存在较大的个体差异,敏感率最高的化疗药物为羟基喜树碱,腹水治疗有效病例15例,其中完全缓解1例,部分缓解14例,总有效率46.9%,不良反应以骨髓抑制为主,有12例(37.5%),腹腔化疗有效者治疗后KPS较治疗前显著升高,差异有统计学意义。结论ATP-TCA法指导大肠癌患者腹腔化疗安全有效,腹水治疗有效者可提高生活质量。

     

    Abstract: ObjectiveTo evaluate the feasibility and efficacy of the intra-peritoneal chemotherapy guided by ATP biolu minescence tumor chemo-sensitivity assay for human colorectal cancer. Methods Thirty-two cases with the colorectal cancer malignant ascites were collected for ATP-TCA to deter mine the chemo-sensitivity. The drug with the highest inhibition rate was selected for intra-peritoneal chemotherapy. The results of ATP-TCA and the response of malignant ascites, the clinical features and Karnofsky performance score(KPS) were analyzed. ResultsThere were considerable differences in chemo-sensitivity between individuals. Hydroxycamptothecin(HCPT) was the most effective to the colorectal cancer malignant ascites. Fifteen patients showed response by intra-peritoneal chemotherapy (complete response: 1, partial response: 14). The KPS of the responders was improved significantly. ConclusionIn vitro chemo-sensitivity ATP biolu minescence tumor assay was safe and effective to guide intra-peritoneal chemotherapy for colorectal cancer malignant ascites, through improvement of the KPS.
